VSP is the UT provider, no forms are required for office visits. Vision care insurance provides for examination, lenses, and frames every 24 months for the employee and spouse. Examination and lenses are provided every 12 months for children (24 months for frames). There is a $10 co-payment charge for each examination and a $15 co-payment for materials.
