    Graduate Assistant Payroll Deduction Instructions
    Graduate assistants on the University of Toledo Main campus are eligible to payroll deduct health insurance.  This process does not apply to students on the Health Science campus and students who are not receiving a graduate assistantship.

    Before completing the Payroll Deduction form, please review the terms for payroll deduction, as well as applicable rate information, instruction for submitting the form, and the enrollment dates.  Questions regarding payroll deduction can be directed to Mary Main, mary.main@utoledo.edu or 419-530-2283.

    Terms of Enrollment

    Enrollment in payroll deduction will only be granted to students who have complete assistantship paperwork filed in the College of Graduate Studies, including appropriate VISA status (international students).  Students stipend amount must adequately cover the health insurance opted for through payroll deduction. Should the student discontinue the graduate assistantship or be removed from payroll before applicable fees have been deducted, the remaining balance will be charged to the student's University of Toledo account and due immediately.  The student is responsible for all fees incurred until such time as that payroll deduction eligibility can be determined by the College of Graduate Studies. The first payroll deduction will not take place until October 10, 2008 for Fall 2008 and February 7, 2009 for Spring 2009.

    University of Toledo Health Insurance

    • Enrollment for this option is only available in the Fall and Spring during the open enrollment period and will be deducted over 6 and 7 bi-weekly pay periods(respectively for Fall and Spring). 
    • Only Graduate Assistants receiving an award may opt to enroll in the payroll deduction plan for health insurance.
    • Coverage of payroll deducted health insurance is deducted by semester (fall and spring) and provides coverage for those terms even if there is a break in enrollment. For coverage information, click here.

    Completing the Payroll Deduction Form

    • The payroll deduction form is set up to work with your default e-mail and will automatically generate and e-mail upon submission.  If you do not have a default e-mail software on your computer, complete the form, right click and save as, rename the form, open your popular email (GMAIL, Yahoo, etc) in another browser and attach the saved file in an e-mail to mary.main@utoledo.edu
    • You can also print, sign and fax or return the form to the College of Graduate Studies. 419-530-4724 (fax) or University Hall 3240 for drop-off.

    Please read and print for your records all terms before completing the Payroll Deduction form.
