    Transcript Evaluation

    Undergraduate transfer credit will be performed upon admission to the University. If you have applied for admission to UT, send your transcripts to:

    Sharon Periat
    Attn: Transcript Review for HIM
    The University of Toledo
    2801 W. Bancroft St., Mail Stop 119
    Toledo, Ohio 43606

     -Or- FAX your information to Sharon Periat at 419-530-5366.

    Include your current address, email address, and specify Review for HIM.

    Please note: It can take up to four weeks following receipt of your transcript to the College of Health Science and Human Service to properly address transfer credit, depending on the number of colleges attended, credits received, degrees awarded, etc. Please allow 2 months following processing of your application to ensure all transfer credit, if any, has been identified and logged on your program of study. Remember to review your program of study, sign and provide to the College Advising Office.

    Undergraduates With Degrees (UWD) should submit a transcript to prove completed baccalaureate degree status. UWDs will need to have comparable courses to meet the two prerequisites for the CHIA program:

    •      Medical Terminology
    •      A Statistical Math course

     Please notify the College Advising Office of the institution that has posted the above two prerequisite courses. 

    Visit www.transfer.org if you want to see a quick overview of your past credits that may transfer. This will provide you with a “mini-snapshot” of what will transfer directly to UT.
